Health Treatment Plans
We would like to take this opportunity to remind you of the need to provide us with the health forms needed for the care of your child at school this year.
If your child experiences any health problems such as asthma, life threatening allergies, seizures, diabetes, sickle cell anemia, or requires a treatment such as tube feedings or suctioning at school, a treatment or emergency plan is required by Prince William County Schools. A new and current plan is required each year.
The forms are available online or by request in the office. Please have these filled out by your doctor at your convenience over the summer time. They are due back to Kerrydale asap for review by your school nurse and when necessary, they will be sent to Student Health Services for final approval.
Your instruction may be requested for personnel training prior to the first day of school. You will be notified if this is necessary. Please contact your school nurse if you have any questions.
